Green stage accidents

I walked into the bathroom and saw that my kitty had not only peed on the floor but pooed on the floor in the same spot right next to the toilet. She has been using the green stage for weeks now. Some times with the seat up and some times with the seat down. We have been reducing the litter little by little. She has never had a problem before so Im not sure what could cause this. We have been doing everything the same. Any suggestions or recommendations? Thank you!

Reducing the litter is one of the hardest thing in the training and should not be done until your cat is really ready for it, I would suggest put as much litter as possible in he green tray and see if that helps - if not - you might need to go back a step or do the potty dance...
